  1. Agreement to Terms

By accessing this website or scheduling services with Optimal Wellness Solutions PLLC, you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, you should not use our website or services.

  1. Services Provided

Optimal Wellness Solutions PLLC offers wellness and medical consultation services, including medical cannabis certification and telehealth evaluations. Our services are provided by licensed nurse practitioners in accordance with state laws and professional standards.

  1. Appointment & Cancellation Policy

A credit card is required to reserve your appointment. Cancellations or reschedules must be made at least 24 hours in advance. Failure to do so will result in a charge of 50% of the appointment fee. If you are not approved for certification, your fee will be fully refunded. No refunds are issued after certification approval, as medical cannabis visits are medical services and are non-refundable once rendered.

  1. Payment & Credit Card Authorization

By scheduling services, you authorize Optimal Wellness Solutions PLLC to charge your card through Stripe. Charges will appear as “Optimal Wellness Solutions PLLC.” This authorization remains in effect until canceled in writing. You agree not to dispute charges for services rendered in accordance with our policies.

  1. Electronic Communication & Telemedicine

You may communicate with our practice via phone, email, portal, or telehealth platforms. While we aim to respond within 24 hours, immediate responses are not guaranteed.

Electronic communications and telemedicine carry inherent risks, including potential confidentiality breaches. By using these services, you acknowledge and accept these risks. Telemedicine does not replace in-person evaluations, and some conditions may require physical examination.

You may withdraw consent for telemedicine at any time; however, this may limit our ability to continue care.

  1. SMS Terms & Conditions

1- SMS Consent Communication: The information (Phone: 508-817-9300, 508-817-9308, 508-365-5208, 508-365-2257) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

2- Types of SMS Communications: If you have consented to receive text messages from Optimal Wellness Solutions PLLC, you may receive messages related to the following:

Steps to Certification

Step 1
Book an appointment

Book an appointment via our website, phone, or email. You must be a Massachusetts Resident

Step 2
Login to HIPAA

Once the appointment is booked, an email is sent with a link to login to our secure HIPAA compliant patient portal where documents are filled out and reviewed by the Nurse Practitioner prior to your visit.

Step 3
Upload Documents
Upload a valid photo ID to the patient portal.

Step 4
Receive Appointment

You will receive appointment reminders before the visit via email, automated voice message, and text.

Step 5
Your visit

Your visit: Telemedicine visits are via a secure HIPAA compliant patient platform. The visit may be done by telephone if preferable. Once connected, the provider will review your medical history and discuss how to incorporate Medical Cannabis into your routine.

Step 6
Invoice

Your invoice is available for payment online in your patient portal. We will process it after your visit. If certification is denied for any reason, we will issue a refund immediately.

Step 7
Here You Go

When the above steps are complete, you will receive two emails. A welcome email is received from the Massachusetts Medical Use of Marijuana Program, and another email from us at Canna Card Express. A temporary program ID card is attached to our email, it is ready to use at dispensaries and is valid for 2 weeks. Our email contains step by step instructions on how to complete the registration online with the state. Registration with the state is required to receive the hard copy Medical Marijuana Patient ID Card. Registration requires a valid MA ID, proof of residency, and is valid for one year. In a year, a renewal visit is needed to keep certification active.
